Required documents

1. Identity Card of the applicant;
2. Documents confirming kin relationship for obtaining renewed certificates about the deceased persons (Birth, wedlock certificate);
3. Receipt of payment of state fee.

Required persons

1. The person himself/herself  about whom the note of relevant civil status act is prepared;
2. Parents, adoptive persons, guardians or guardian and foster care agencies about minors; nurture, treatment, and social protection entities of the population;
3. Relatives of the dead person;
4. Authorized persons on the basis of a notarized power of attorney.

State Fee

2 AZN state fee is paid for the issuance of certificates of the state registration of civil status acts (renewed certificates).
