1. Joint application of a child’s father and mother (parents) for determination of paternity;
2. ID cards of applicants;
3. Birth certificate of the child;
4. Resolution that legally entered into force in case of paternity is determined by the court;
5. Along with the application of child’s father in case of death of the mother, consideration of incapable, impossible to identify the place of residence of mother or deprivation of parental rights;
- The document confirming the consent of entities of guardianship and trusteeship;
- Certificate about the death of the mother (copy)
- Extract from the resolution of the court legally entered into force about the consideration of the incapability of the mother;
- Extract from court resolution legally entered into force about deprivation of mother from parental rights;
- Extract from court resolution legally entered into force about unaware disappearance of mother or document confirming impossibility to identify her place of residence should be presented;
6. In case of determination of paternity on adult person his/her consent, in case of consideration the incapability of the person the consent of his/her guardian or the consent of guardianship and trustee entity of executive power.
