Issuance and renewal of identity card, as well as the inclusion of the information into chip carrier and renewal or use of such information
1. The following documents are required for the issuance of ID card for the first time:
· application form for the issuance of ID card ( Annex 1 - is compiled electronically by a responsible employee of the Ministry of Internal Affairs) ;
· Birth certificate of a citizen, or personal identification card;
· The document, which provides the basis for moving to a living area as specified in Article 2 of the Statute on the Application of the Law on “Registration on the place of residence and place of stay" approved by the Law of the Republic of Azerbaijan No 773-IQ dated December 8, 1999 (if a place of residence of a citizen is not registered)
· medical certificate about blood group, eye color, medical certificate about height;
· document on payment of the state fee;
· if the application is issued by the adoptive parent, guardian or custodian of a citizen - a document certifying his/her rights.
· application of an applicant under 18 years may be submitted either by himself or his legal representative. In both cases, the application must contain a signature of a citizen between the ages of 15 and 18.
2. Fingerprinting of citizens, as well as taking a photo in accordance with the requirements of the International Civil Aviation Organization (ICAO) shall be made free of charge in the issuing institution.
3. Issuance of certificates related to electronic signature, creation of electronic signature and issuance of verification information, as well as suspension, restoration or annulment of those certificates shall be carried out in accordance with the Law of the Republic of Azerbaijan "On Electronic Signature and Electronic Documentation".
4. Identity card is issued for 10 years, whereas for persons reached the age of 55 – for an unlimited period of time. When the validity of the ID card is expired or the citizen's last name, first name, patronymic name is changed; when the ID card, including its chip carrier becomes useless or the information on the card is found to be incorrect a citizen should apply to the issuing authority for the change of the identity card within 1 (one) month.
5. If the identity card is changed due to its expired validity, or change in the citizen's last name, first name, patronymic name, as well as the uselessness of the ID card, including its chip carrier, along with taking a photo and fingerprinting of a citizen - the following are required:
· application form for the renewal of ID card ( Annex 2 - is compiled electronically by a responsible employee of the Ministry of Internal Affairs) ;
· identity card;
· document on payment of the state fee;
· if the application is issued by the adoptive parent, guardian or custodian of a citizen - a document certifying his/her rights.
6. If unchangeable information provided on the identity card and included in the chip carrier is incorrect, it is changed within one day from the date of submission without paying the state duty. If changeable information provided in the chip carrier of the identity card is incorrect, the data will be changed immediately (within 30 minutes at the latest) after submission of the incorrect information to the body that has entered the information without a state fee.
7. When receiving a new identity card, a citizen must hand over the old identity card.
NOTE: When it is necessary to determine a person's citizenship of the Republic of Azerbaijan, he/she may apply to the relevant service of the State Migration Service of the Republic of Azerbaijan provided at “ASAN service” center.
Loss or theft of the identity card
· In case a citizen loses his/her identity card, he/she must provide written information to the issuing authority indicating where, when and under which circumstances the incident occurred.
· If a citizen informing on his lost identity card, finds a lost identity card after being issued a new ID card, the found identity card must immediately be handed over to the body that issued it.
· In case the identity of a citizen writing an application to the issuing authority on the loss of the identity card is determined, this information is transmitted to the state registry of invalid documents. After the information on the lost identity card is included in the state registry of invalid documents, this document is considered to be invalid, as well as active status in IAMAS is changing. After the state fee for re-issuance of identity card due to its loss is paid and the application for the issuance of the identity card is filled out, a new identity card is issued to a citizen.
· When the identity card is stolen, a citizen should write an application to the police office located in the place the crime occurred. Citizens are issued a reference on acceptance of such application and this reference shall be considered as the basis for the registration of the stolen identity card in the state registry of invalid documents and the issuance of a new identity card to a citizen.
· Identity card of the citizen of the Republic of Azerbaijan shall be issued in accordance with the procedure established by the relevant bodies of the Ministry of Internal Affairs of the Republic of Azerbaijan (hereinafter - the issuing authority) and with the Law of the Republic of Azerbaijan "On Identity Card of the Citizen of the Republic of Azerbaijan" and Statute on the application of the Law of the Republic of Azerbaijan "On Identity Card of the Citizen of the Republic of Azerbaijan".
For the issuance of new generation ID cards to citizens of the Republic of Azerbaijan who reached the age of 15:
· issued or renewed within ten working days – 5 AZN;
· issued or renewed within three working days – 25 AZN;
· issued or renewed within one working day – 35 AZN.
For the issuance of new generation ID cards to citizens of the Republic of Azerbaijan under the age of 15:
· issued or renewed within ten working days – 4 AZN;
· issued or renewed within three working days – 20 AZN;
· issued or renewed within one working day – 25 AZN.
For re-issuance related to the loss or damage of the new generation ID cards to the citizens of the Republic of Azerbaijan:
· issued or renewed within ten working days – 15 AZN;
· issued or renewed within three working days – 35 AZN;
· issued or renewed within one working day – 45 AZN.
