Application –questionnaire – 2 sets; 
Application in the name of the President of the Republic of Azerbaijan – 2 sets; 
Curriculum – vitae – 2 sets; 
Four photos size 3x4 cm; 
Receipt proving payment of the state duty;
Certificate from the Ministry of Education proving proficiency in the official language of the Republic of Azerbaijan; 
A copy of the permit card for permanently residing in the territory of the Republic of Azerbaijan or a refugee card issued to persons granted refugee status ( the persons determined by a court to be stateless or permanently residing in the Republic of Azerbaijan are not requested such cards); 

A notary certified copy of the passport or any other border–crossing document ( except  the persons granted refugee status without the same, including those who are determined by a court to be stateless or permanently residing in the Republic of Azerbaijan); 

A notary certified copy of the birth certificate ( to be translated if not available in Azerbaijani language); 

Subject to Section 4, Clause 14, Law “On citizenship of the Republic of Azerbaijan” of the Republic of Azerbaijan, a copy of the court judgment proving statelessness or permanent residence of an individual in the Republic of Azerbaijan; 

A document proving a legal source of income exists ( one of the following documentation: certificate from the workplace ( by stating the salary), pension, scholarship or targeted state social aid certificate, a certificate from the tax authority on incomes of a physical person, alimentary pay certificate, certificate on the income of a person under guardianship, certificate of inheritance and etc.) or: 

A notary certified copy of a guardian’s guardianship certificate;
a notary certified copy of a guardian’s ID Card; 
A certificate from the guardian’s workplace; 

A certificate received from the Embassy in AR of the country regarding the fact that the stateless persons born in Azerbaijan are not citizens of their country of residence ( to be translated if not available in Azerbaijani language); 
For persons with the highest achievements in science, technology, culture or sports fields – a substantiated petition of the government agency having invited him/her to the Republic of Azerbaijan; 

Copies of ID cards of their near relations (if any) who are citizens of the Republic of Azerbaijan

Nationalization of the persons residing in the Republic of Azerbaijan (if having a child)

Application in the name of the President of the Republic of Azerbaijan – ( by stating children’s names)-2 sets; 
Curriculum vitae – 2 sets; 
 Four photos size 3x4 cm (for the individual himself and his children); 
Receipt proving payment of the state duty;
Certificate from the Ministry of Education proving proficiency in the official language of the Republic of Azerbaijan; 
A copy of the permit card for permanently residing in the territory of the Republic of Azerbaijan; 

A notary certified copy of the passport or any other border–crossing document ( individual himself and his children); 

A notary certified copy of the birth certificate ( to be translated if not available in Azerbaijani language);
