• Application questionnaire – 2 pieces;
• Application in the name of the President of the Republic of Azerbaijan (provided that the reason for renunciation of citizenship is indicated) 2 pieces;
• Biography – 2 pieces;
• Four photos 3x4 cm in size;
• Receipt on payment of state duty;
• Copy of an identity document;
• A document from the Azerbaijan Credit Bureau on the absence of credit obligations;
• A document from the State Tax Service under the Ministry of Economy of the Republic of Azerbaijan on the absence of tax liability to the state;
• A document from the relevant local department of the Executive General Department of the Ministry of Justice of the Republic of Azerbaijan regarding the nonwithholding of alimony and the absence of any enforcement document;
• A document on the absence of military service obligation (notarized copy of the military ticket or a certificate from the local department of the State Service for Mobilization and Conscription where it is registered);
• A certificate from the housing operation area at the place of residence stating that there is no debt for housing utilities;
• A certificate from the relevant local branch of "Azersu" OJSC stating that there is no debt due to the use of water and the discharge of wastewater;
• Certificate of no debt due to gas usage;
• Certificate of no debt due to the use of electricity;
• Certificate of no debt due to telephone conversations.
