1. The application questionnaire, biography and application written in the name of the President of the Republic of Azerbaijan should be printed electronically.
2. A medical certificate of not being a carrier of infectious diseases can be obtained from any medical institution separately or in general and submitted (if the persons applying for restoration of citizenship have not left the republic, the mentioned certificate is not required from them).
