Required documents

• an application;
• if the applicant is a legal entity - state registration certificate;
• if the applicant is a natural person - a copy of the identity document;
• a copy of the certificate of registration of the applicant as a taxpayer;
• a certificate submitted by persons who received a special permit for the circulation of items containing ozone-depleting substances, stating that the item does not contain ozone-depleting substances prohibited for circulation by the Montreal Protocol (Material Safety Data Sheet passport (MSDS) is sufficient);
• a document certifying that the number of ozone-depleting substances, whose circulation is not prohibited by the Montreal Protocol, does not exceed the limits set by that Protocol (it is sufficient to submit a notarized translation of the purchase agreement and the invoice for the imported product);
• a certificate on the method of disposing of the ozone-depleting substance contained in the item with a special permit after the end of its useful life;
• a program of appropriate measures to prevent an accident;
• a copy of documents on specialists having appropriate qualifications.
