- for legal entities - name, legal address, type of entrepreneurial activity;
- for individuals - name, patronymic, surname, identity card information (series, number, when and by whom it was issued, address), type of entrepreneurial activity:
- a copy of the relevant document on the tax registration of the applicant by the tax authorities;
- a copy of the license in cases provided by the legislation;
- the opinion of the relevant executive authority in the cases provided for in the legislation;
- a document on the presence of appropriate equipment intended for the extraction of precious stones, gold and other precious metals, oil, oil products and natural gas, their processing, oil and oil product waste;
- a confirmation document (quality and compliance certificate) that the means of production and processing meet the regulatory and technical requirements;
- a copy of the documents on the specialists having the relevant qualifications;
- a document on the allocation of the relevant land area and subsoil for the purpose of extraction of precious stones, extraction of gold and other precious metals, oil, natural gas, and processing of oil product waste;
- a document on the organization of the protection of the relevant objects intended for the extraction of precious stones, the extraction of gold and other precious metals, oil, oil products and natural gas, and the storage of the means for the processing of oil product waste;
- copies of geological, technical, and marking documents carried out by persons in the process of extraction of precious stones, extraction of gold and other precious metals, oil, natural gas, and processing of oil product waste;
- a document on ensuring the safe performance of work related to the extraction of precious stones, extraction of gold and other precious metals, oil, natural gas, and processing of oil product waste;
- a program of appropriate measures to prevent an accident situation.
