1. Identity cards of parents;
2. Wedlock certificate of parents’;
3. Birth certificate of parents’;
4. A document from a medical institution confirming the birth fact, time, and place;
5. In case of a baby is born out of a medical institution, a document issued by a doctor who assisted for birth or a doctor that parents applied to after birth;
6. In case of a baby born dead, a document given by a doctor about the chi born;
7. In case of a baby had died before the time mother checked out of the medical institution, a document issued by the doctor confirming birth and death;
8. Regarding found children, a document confirming time, place, and condition regarding the children are found (act, protocol, reference, and a document issued by medical institution proving age of the child);
9. Court resolution that legally entered into force about the determination of birth fact;
10. In case of the prescribed period of 1 month for the registration of birth has expired, reference by doctor on the health of the baby;
1. Parents or one of them;
2. If parents cannot apply for a valid excuse, their relatives, neighbors, administration of the medical institution where a baby was born, or authorized persons based on notarized power of attorney
According to the Administrative Offences Code of the Republic of Azerbaijan, in case of parents do not apply for registration of birth during 1 month period without any valid excuse, the child’s parents are warned or fined for an amount 10 AZN.
