1. Application about changes to the given name, patronymic and family name;
2. Birth certificate of applicant;
3. If an applicant is married, wedlock certificate;
4. If an applicant is divorced, divorce certificate;
5. If an applicant has children, their birth certificates;
6. The notarized copy of the ID card;
7. Notarized copy of card about attachment to summons point or a notarized copy of military card;
8. 2 photos of an applicant in the size of 3x4sm
9. Receipt of payment about the state fee.

Required persons

1. Parents of the child until the age of 18;
2. Applicant himself(herself);
3. Authorized persons on the basis of a notarized power of attorney.
