The original of the military card on exemption from military duty at the age of 18-35 or the card on assignment to the conscription station on the note on granting the right to deferment.
If the passport is issued within two working days due to urgent treatment of a citizen of the Republic of Azerbaijan or a person (persons) traveling with him, or serious illness or death of a close relative (parent, husband (wife), child, brother, sister, their children, grandfather, grandmother, grandson, grandfather (grandmother) father or mother, uncle, aunt, their children, father - in-law, mother-in-law, brother-in-law, sister-in-law) living abroad, these facts must be verified by one of the following documents:
letter from the health authority confirming the need to be treated urgently;
a letter from a medical institution of a foreign country about the need for urgent admission to treatment (at the same time, a medical opinion of a medical institution at the place of residence of the patient must be submitted);
certificate of a medical institution of a foreign country confirming the fact of a serious illness or death of a relative.
When a passport is issued to a citizen under the age of 15:
a document confirming the rights of the adopter, guardian or custodian of children deprived of parental care.
When the validity period of the passport of a citizen expires, or the surname, name, patronymic of a citizen changes, or the passport, including its electronic carrier (chip) becomes unusable, the passport must be changed. Changing the passport is carried out based on the following documents, with a photograph of citizen taken at the authority issuing the passport, but without requiring a medical certificate of the blood group:

When the passport of a citizen under the age of 15 is changed:
A document confirming the rights of the adopter, guardian or custodian of children deprived of parental care;

- for children from three to eighteen years old-35 Manats;
- to citizens who have reached the age of eighteen-60 Manats.

- citizens who have reached the age of eighteen-120 Manats
when issued or changed within one business day:
- for children under one year-55 Manats;

- citizens who have reached the age of eighteen-210 Manats.
For re-issuance of passports of citizens of the Republic of Azerbaijan in connection with the loss or deterioration of their passports giving the right to travel abroad:
when issued within ten working days
- for children under one year - 25 Manats;
- for children from one to three years old-40 Manats;
- for children from three to eighteen years old-55 Manats;
- citizens who have reached the age of eighteen-75 Manats.
when issued within five business days
- for children under one year-40 Manats;
- for children from one to three years old-70 Manats;
- for children from three to eighteen years-100 Manats;
- citizens who have reached the age of eighteen-135 Manats.
when issued within one business day
- for children under one year-65 Manats;
- for children from one to three years old-125 Manats;
- for children from three to eighteen years-165 Manats;
- citizens who have reached the age of eighteen-220 Manats.
When a citizen under the age of 15 is issued the passport, the application form is signed only by his parents, and if it is issued to the children deprived of parental care, the application form is signed by other legal representatives.
The application form of a citizen aged 15 to 18 years can be submitted by him or his legal representative. In both cases, the application-questionnaire must contain the signature of a citizen aged 15 to 18 years.
A regular passport is issued for children under 1 year for a one-year period, for children under 1 year and 3 years for a three-year period, for children under 3 years and 18 years for a five-year period, and for persons over 18 years of age for a ten-year period.
Taking fingerprints of a citizen, as well as photographing him in accordance with the requirements of the International Civil Aviation Organization (ICAO) is carried out in the body issuing the passport. No fingerprints are taken from citizens under the age of 15.
The legal representative submits documents for the issuance of a citizen's passport to a citizen deemed incapacitated by the court. In this case, the person must also submit documents confirming the right to legal representation.
When changing passports of a citizen who does not have an electronic carrier, fingerprints of a citizen are taken.
The expired passport is handed over to the relevant state body.
If an error is identified in the passport of a citizen, including the information specified in his electronic carrier, it is changed within one day from the date of submission of the passport without charging the state fee.
When issuing and changing passports, photographs are taken using special equipment within the center.
