  • An identity card of the applicant;
  • Power of Attorney (if an applicant is not a close relative).

When the father or mother applies – an ID card shall be required;

When the wife applies – the marriage certificate and identity card shall be required;

When children, brothers, and sisters apply - a birth certificate and an identity card shall be required.
